ReDistribution of Per Capita Distribution pursuant to Article 4.5 of the Red Pheasant Cree Nation Sovereign Wealth Fund Trust Agreement (“the Trust Agreement”)

Distributions to Red Pheasant Minors: (a) Upon receipt of a Council Resolution or other evidence confirming that a Red Pheasant Minor who was alive on the Ratification Date has reached the age of eighteen (18) years old, the Trustee is authorized to pay each Red Pheasant Minor from the Trust Account the sum of fifteen thousand dollars ($15,000) plus accrued interest of 4% compounded annually from the Compensation Date.

Pursuant to article 4.5 of the Trust Agreement, I

We strongly encourage direct deposit. If you cannot provide banking information for direct deposit, please email pcd@redpheasantcn.ca to discuss how a bank account could be opened for you.

3. Form B · Direct deposit information

Please provide the following personal information and attach a void cheque or a standard bank direct deposit form. This information will be used for the sole purpose of depositing funds electronically to the account described below. BMO complies with applicable privacy legislation, including the Personal Information Protection and Electronic Documents Act (Canada) and BMO’s privacy code is posted at www.bmo.com.

Please type it as day/month/year, for example 01/08/1990.
The 5 digit number on your cheque, also called the branch number.
The 3 digit number on your cheque, also called the financial institution number.
Numbers only, from your cheque or online banking.
Type it again to make sure it is right.

How your information is handled

What you enter here is held by Red Pheasant Cree Nation on the Nation's own system. Only designated staff can read it. It is used to make your per capita distribution payment, and your banking details go to BMO Trust Company for that purpose only. You can ask to see or correct your information at any time by calling the band office.
